Issuers approve or decline transactions partly on the data sent with the authorization request. If the merchant category code, transaction date, location or other fields were wrong, the issuer may have approved a transaction it normally blocks, for example a gambling or high risk category sent as general retail. Code 12.7 lets the issuer return that transaction. It is a processing error dispute, so the cardholder usually never questioned the purchase.
